The system thinks it updated, but the build number and version never change. Because I’m stuck on 3.0 Build 23219, the CarPlay preferences menu never appears, and plugging in my iPhone does nothing.

If you stay with Sync 3.0 you need to update from version 1 to version 2.2/2.3 first, then to 3.0.23219 or so.
If you reformat the APIM from any version of Sync 3.0 you do not need the interim step.
